A Summary of Your Rights  Under the Fair Credit Reporting Act (FCRA)

The federal Fair Credit Reporting Act (FCRA) is designed to promote accuracy, fairness, and privacy of information in the files of every "consumer reporting agency" (CRA). Most CRAs are credit bureaus that gather and sell information about you (e.g. if you pay your bills on time or have filed bankruptcy), to creditors, employers, landlords, and other businesses. The FCRA gives you specific rights, as outlined below. You may have additional rights under state law. You may contact a state or local consumer protection agency or a state attorney general to learn those rights.

  • You must be told if information in your file has been used against you.
  • You can find out what is in your file.
  • You can dispute inaccurate information with the CRA. Inaccurate information must be corrected or deleted.
  • You can dispute inaccurate items with the source of the information.
  • Outdated information may not be reported.
  • Your consent is required for reports that are provided to employers, or reports that contain medical information.
  • You may choose to exclude your name from CRA lists for unsolicited credit and insurance offers.

In accordance with the requirements of the FCRA, I hereby consent and authorize that a credit report, consumer report and/or investigative consumer report be obtained in connection with my employment with Voorhees or prospective employment with Voorhees. I understand that these investigative background inquiries may include credit, consumer, criminal, driving, prior employment and other reports. These reports may also include information as to my character, work habits, performance and experience, along with reasons for termination of past employment from previous employers.  I also understand that Voorhees and its agent(s) may request information from Federal, State and other agencies relative to my driving, credit, criminal, as well as other experiences.  

By submitting this application you:

  • Consent and authorize Voorhees and its agent(s) to obtain the aforementioned reports for employment purposes. Also release Voorhees, its agent(s), and persons and organizations providing information from all claims and liabilities of any nature that may result from this research.  I also authorize photo copies and facsimiles of this authorization form to be considered as originals, if necessary, while researching my background.
  • Certify that the information contained on or submitted with this application is true, accurate and complete; and understand that if employed, any misrepresentation or material omission of facts on this application or other employment documentation may result in a retraction of any job offer or immediate termination of employment.
  • Authorize Voorhees College to contact your former employers, except where indicated otherwise, and your schools for a complete account of their experiences with you; and unconditionally release all parties from liability from any damage that may result from furnishing your employment-related information.
